Printed on a card stock like a post-card (thick paper that feels good).
Approximate size almost: 6 x 4 inches.
Important note: If you prefer sign-up forms to be larger and more legible (1/2 of an 8.5×11 inch page = 8.5×5.5 inches each) and one-sided, instead of ordering these cards, just photocopy the page with 2 copies of forms on our webpage and then cut the page in half to make two single-sided sign-up forms: Vital Coordinator & Parish Resources database & Manual – Apostolate of Eucharistic Adoration Inc.
Choose your language (English, French of Spanish). If you want another language, send us your translation and we will make it.
Double sided. For single-sided, see the other product.
Choose horizontal/landscape or vertical for the direction of the images on the cards.
Side 1 – choose the sign-up form you want (options are with 4 time quadrants or listing days and hours or customized). If you select the option for a customized sign-up form, you must call or email us specifying the time zones or days and hours you want on the sign-up form or any other changes: 1-800-784-9550 info@perpetualeucharisticadoration.com
Side 2 – choose the image with an inspiring message for the other side. This can also be customized upon request.
